In the current antibiotic resistant era, TR biopsies of the prostate have alarmingly high infectious complications and readmissions. The TP route is a credible step to minimise these events, however the data comparing the two approaches has not been consolidated. METHODS This is an evaluation of TR versus TP biopsy approach in the context of 28 days post procedure complications and readmissions, with secondary evaluation of burden of expenditure in NHS hospitals over the entire decade (2008-2019) by evaluating national Hospital Episode Statistics (HES) data. RESULTS In this data set of 486,467 prostate biopsies (387,879 TR and 98,588 TP), rates for infection and sepsis were higher for the TR compared to the TP cohort (0.53% vs 0.31% p less then 0.001 CI 99%). Rates of sepsis have more than doubled for TR bOligonucleotides incorporating a central C-nucleoside with either rigid or flexible benzaldoxime base moiety were synthesized and the hybridization properties of their metallacyclic derivatives studied by UV melting experiments. In all cases, the metallated duplexes were less stable than their unmetallated counterparts and the metallacyclic nucleobases did not show a clear preference for any of the canonical nucleobases as a base pairing partner. With palladated oligonucleotides, increased flexibility translated to less severe destabilization whereas the opposite was true for the mercurated oligonucleotides, reflecting the greater difficulties in accommodating a rigid Pd(II)-mediated base pair than a rigid Hg(II)-mediated base pair within the base stack of a double helix. © 2020 WILEY-VCH Verlag GmbH & Co. This study compares the incidence of infective complications after two different durations of perioperative antibiotic treatment in patients undergoing surgical reconstruction with skin dermal substitutes (SDS) after excision of skin cancer. Infective complications at the site of SDS were compared in subjects undergoing surgical reconstruction who received either a?>?24-hour (extended protocol) or a???24-hour (short protocol) perioperative antibiotic treatment. Of 116 patients undergoing SDS surgical reconstruction, 62 (53.4%) received an extended schedule, and 54 (46.6%) received a short schedule. The two groups were similar for gender, age, comorbidities, American Society of Anesthesiologists score, and type of skin cancer. Overall incidence rate of infection was 20.7% (24/116). No differences in terms of risk of infection were observed between the two groups (OR 1.04, 95% CI 0.42-2.55; P =?.9BACKGROUND Inflammation has an important role in cancer survival, yet whether serum markers of inflammation predict response to potentially curative neoadjuvant chemotherapy (NAC) in oesophageal adenocarcinoma (OAC) is controversial. This study aimed to determine whether the systemic inflammatory response (SIR) is associated with response to NAC and survival. METHODS Consecutive patients with OAC planned for surgery with curative intent received blood neutrophil and lymphocyte measurements at diagnosis to calculate the neutrophil to lymphocyte ratio (NLR). Pathological variables including pTNM stage, differentiation, vascular invasion and Mandard tumour regression grade (TRG) were recorded. TRGs 1 and 2 were taken to represent a good response, and the primary outcome was overall survival. RESULTS During follow-up of 136 patients, 36 patients (26?5 per cent) had recurrence and 69 (50?7 per cent) died. Receiver operating characteristic (ROC) curve analysis of NLR before NAC predicted poor TRG (area under the ROCOX15 mutations were shown to underlie Leigh syndrome (LS), a progressive subacute necrotizing encephalopathy caused by defects in the mitochondrial respiratory chain. Here, two siblings of consanguineous kindred presented in infancy with a syndrome of hypotonia, nystagmus, psychomotor retardation, and pyramidal signs. Toward the end of their second year, both patients developed progressive quadriparesis, convulsions, and pseudobulbar palsy. Similar to two previously reported cases, one of the two affected siblings had severe hypertrophic obstructive cardiomyopathy, hearing loss, and no visual response. Through linkage analysis and whole-exome sequencing, we identified a homozygous p.R217W mutation in Cytochrome C oxidase assembly protein COX15 homolog. Consistent with the known heterogeneity of mitochondrial diseases in general and that of LS in particular, several phenotypic features were markedly distinguished between the affected siblings and in relation to previous reports of COX15 mutations. InterestingBACKGROUND Stress-related dissociation has been shown to negatively co-vary with pain perception in current borderline personality disorder (cBPD). While remission of the disorder (rBPD) is associated with normalized pain perception, it remains unclear whether dissociation proneness is still enhanced in this group and how this feature interacts with pain sensitivity. METHODS Twenty-five cBPD patients, 20 rBPD patients and 24 healthy controls (HC) participated in an experiment using the script-driven imagery approach. We presented a personalized stressful and neutral narrative. After listening to the scripts, dissociation and heat pain thresholds (HPT) were assessed. RESULTS Compared to HC, cBPD patients showed enhanced dissociation and exhibited significantly enhanced HPT in the neutral condition, whereas rBPD participants were in between. After listening to the stress script, both clinical groups exhibited enhanced dissociation scores. Current BPD participants responded with significantly higher HPT, whereasBACKGROUND Cancer cachexia is a poorly understood metabolic consequence of cancer. During cachexia, different adipose depots demonstrate differential wasting rates. Animal models suggest adipose tissue may be a key driver of muscle wasting through fat-muscle crosstalk, but human studies in this area are lacking. We performed global gene expression profiling of visceral (VAT) and subcutaneous (SAT) adipose from weight stable and cachectic cancer patients and healthy controls. METHODS Cachexia was defined as >2% weight loss plus low computed tomography-muscularity. Biopsies of SAT and VAT were taken from patients undergoing resection for oesophago-gastric cancer, and healthy controls (n = 16 and 8 respectively). RNA was isolated and reverse transcribed. cDNA was hybridised to the Affymetrix Clariom S microarray and data analysed using R/Bioconductor. Differential expression of genes was assessed using empirical Bayes and moderated-t-statistic approaches. This study evaluated the distribution of the crizotinib patient information brochure (PIB) in Europe and patient knowledge of the key messages in the PIB. A cross-sectional survey was conducted in 10 European countries among patients who received crizotinib to ascertain whether each patient received and read the PIB, and his/her knowledge of its key messages on hepatotoxicity, interstitial lung disease/pneumonitis, QTc prolongation, bradycardia, and vision disorders. Of the 341 patients contacted, 40 responded (11.7%), and 39 patients were eligible. A total of 77% of respondents acknowledged receiving the PIB, of which, 93% reported reading it. Knowledge of the individual side effects ranged from 36% to 85%, and precautions for use ranged from 56% to 67%. Understanding the reasons for calling a physician ranged from 54% to 85%. Knowledge of each of the 6 key side effects was greatAn emerging body of research has investigated the relationship between helping (as a type of organizational citizenship behavior) and emotional exhaustion (as an aspect of employee health). Research has demonstrated a significant relationship between helping and emotional exhaustion, but the theoretical arguments for the causal direction vary across studies. Specifically, some researchers have conceptualized helping as an outcome of emotional exhaustion, while others have regarded helping as a predictor of emotional exhaustion. This lack of theoretical clarity in directionality hinders the field's ability to summarize existing empirical findings cohesively and elucidate theoretical mechanisms. Therefore, this study attempts to clarify the theoretical directionality between helping and emotional exhaustion using four waves of data collected at six-month intervals. Autoregressive cross-lagged analyses with auto-correlations revealed that more helping was associated with less future emotional exhaustion from WavBACKGROUND The development of lung cancer is a multifactorial process that involves the environmental and genetic factors. The mortality rate of this cancer is higher than breast, colorectal, and prostate cancers. In this study, we try to analyze the proteome of patients with Non-Small Cell Lung Cancer (NSCLC) and compare it with the healthy samples. METHODS This study has compared 30 lung tissue samples from patients with NSCLC and 30 healthy samples using proteomics and RT-PCR. Hence, tissue samples were obtained from the surgical ward in sterile conditions, and then, protein extraction applied to them. At the next stage, two-dimensional electrophoresis and mass spectrometry LCMS/MS were performed for protein isolation and sequencing, respectively. RESULTS The proteome analysis identified more than 40 differences in proteomic pattern of normal lung tissues compared to lung tissues with NSCLC. Peroxiredoxin, Haptoglobin, and Alpha-1 antitrypsin proteins were identified. This study aimed to investigate thyroid-related ICM administration practices among diagnostic imaging departments in Norway. METHODS The cross-sectional survey included 24 hospitals and 75 respondents with a 69% response rate. The survey covered practices for assessment and management of at-risk patients and the participants' perceived rationale for the routines. RESULTS The use of written checklists as recommended by international guidelines was quite modest (15%) and the respondents preferred various methods to identify risk and contraindications. Only 20% reported checking for any scheduled thyroid-scintigraphy and/or radioactive-iodine therapy. 42% indicated that they did not have thyroid-related ICM routines,AIM The relationship between family behaviour and depression in adolescents at clinical high risk (CHR) for psychosis remains understudied despite high rates of depression in this population. This study examines the relationship between family problem-solving behaviours and depression in CHR adolescents and the impact of family interventions targeting subthreshold symptoms of psychosis on reducing symptoms of depression over 2-years. METHODS Participants were a subset of the North American Prodrome Longitudinal Study who were randomized to 6-months of family focused therapy for individuals at CHR or family psychoeducational treatment. We evaluated the relationship between communication during family conflict discussion and adolescents' symptoms of depression before treatment. At follow-up assessments the family treatment groups were compared on depression. Finally, we compared those in family treatment with matched controls. RESULTS Adolescents' constructive communication was associated with less severe symptOur objective was to examine the trends and variation in opioid prescribing in Estonia from 2011 to 2017. This retrospective cross-sectional study is based on a nationwide prescription medicines database. We stratified the analysis by treatment indication (cancer vs noncancer pain). Between 2011 and 2017, annual opioid prescribing rates increased by 67% (from 82.9 to 138.6 prescriptions per 1000 population). The annual number of prescriptions per patient did not change substantially (from 2.94 in 2011 to 2.87 in 2017), and was higher among cancer patients (5.07 vs 2.67 annual prescriptions per cancer and noncancer patients, respectively, in 2017). The use of the most potent opioids (morphine, fentanyl) was higher in noncancer than in cancer patients. The use of prescription opioids is low, and raises concern about the potential undertreatment of cancer pain, in parallel with misuse of opioids for either noncancer pain or diversion. © 2020 The Authors. Compared to a non-halogenated azobenzene, ortho-functionalized tetrachlorinated azobenzenes are capable of red-shifting the  π -> π*  transition from the ultraviolet  (UV) portion of the electromagnetic spectrum into the visible range.  Within this visible range, the azobenzene molecule can be reliably converted from trans to cis with red light (660 nm), and converted back to trans from cis with violet wavelength light (410 nm) and/or thermal relaxation. We also report the gene-silencing ability of these Cl-siRNAzos in cell culture as well as their reversible control with visible light for up to 24 hours. © 2020 WILEY-VCH Verlag GmbH & Co. It is often assumed that manic patients lack insight and cannot accurately report their symptoms. This delayed the development of self-rating scales for mania, but several scales now exist and are used in research. Our objective is to systematically review the literature to identify existing self-ratings of symptoms of (hypo)mania and to evaluate their psychometric properties. METHODS PubMed?, Web of Knowledge, and Ovid were searched up until June 2018 using the keywords "(hypo)mania," "self-report," and "mood disorder" to identify papers which included data on the validity and reliability of self-rating scales for (hypo)mania in samples including patients with bipolar disorder. RESULTS We identified 55 papers reporting on 16 different self-rating scales claiming to assess (hypo)manic symptoms or states. This included single item scales, but also some with over 40 items. Human P-glycoprotein (P-gp) is a multispecific drug-efflux transporter, which plays an important role in drug resistance and drug disposition. Recent cryo-electron microscopy structures confirmed its rotationally symmetric architecture, which allows dual interaction with ATP and substrates. We here report the existence of two distinct, symmetry-related outer gates. Experiments were aided by availability of the X-ray structure of a homodimeric eukaryotic homolog of P-gp from red alga (CmABCB1), which defined the role of an apical tyrosine residue (Y358) in outer gate formation. We mutated analogous tyrosine residues in each half of the human full-length transporter (Y310, Y953) to alanine. These mutants were introduced in engineered transporters which bind rhodamine 123 in one of two symmetry-related binding modes only. Outer gate dysfunction was detected by a loss of active transport characteristics, while these mutants retained the ability for outward downhill transport. Our data demonstrate that symmetric tAIM In patients with Alzheimer's disease (AD), cognitive impairments cause a progressive reduction in Activities of Daily Living (ADL). Neuropsychiatric symptoms (NPS) also appear in most patients; however, the association between NPS and reductions in ADL remains unclear. The present study evaluated whether NPS influence such reductions using two different ADL measures in patients with AD. METHODS Among 546 consecutive outpatients who visited the memory clinic at the Jikei University Kashiwa Hospital, we recruited 208 patients with AD and investigated the correlations between either the Physical Self-Maintenance Scale (PSMS) score or the Instrumental ADL (IADL) level, and each of the Behavioral Pathology in AD (Behave-AD) subscales. To clarify the causal relationships of these correlations, we then verified the associations between statistically significant demographic variables and the Behave-AD subscales as well as the two ADL scales (PSMS score and IADL percentage) using a general linear model. RESULTS NeOBJECTIVES Mismatch negativity (MMN), an auditory event-related potential sensitive to deviance detection, is smaller in schizophrenia and psychosis risk. In a multisite study, a regression approach to account for effects of site and age (12-35?years) was evaluated alongside the one-year stability of MMN. METHODS Stability of frequency, duration, and frequency?+?duration (double) deviant MMN was assessed in 167 healthy subjects, tested on two occasions, separated by 52?weeks, at one of eight sites. Linear regression models predicting MMN with age and site were validated and used to derive standardized MMN z-scores. Variance components estimated for MMN amplitude and latency measures were used to calculate Generalizability (G) coefficients within each site to assess MMN stability. Trait-like aspects of MMN were captured by averaging across occasions and correlated with subject traits. RESULTS Age and site accounted for less than 7% of MMN variance. G-coefficients calculated at electrode Fz were stable (G = 0.6OBJECTIVES A new wireless spinal cord stimulation (SCS) technology, which was introduced in recent years, promises minimal invasive SCS as well as additional advantages such as a wide range of stimulation paradigms and 3-T magnetic resonance imaging (MRI) conditionality. MATERIALS AND METHODS We prospectively evaluated 12 patients suffering from therapy-resistant neuropathic pain, who were implanted with a wireless SCS system from 2017 to 2019. Potential issues pertaining to handling and usability of the SCS device were evaluated from a patients' as well as from a surgeon's perspective. RESULTS Mean follow-up was 228.0?days (95% CI, 20.0-518.0?days). We did not record any handling issues nor did we record any relevant local discomfort associated with the implanted SCS device. N = 3/12 patients reported discomfort from wearing the SCS antenna and one patient complained about a short battery life of the controller device. There were no reported incidents during 3-T MRI studies. After an average test period of 5Zika virus (ZIKV) is an enveloped, positive single-stranded sense RNA virus transmitted by Aedes species. Many efforts have been conducted to find a good, reliable and cost-effective test for ZIKV diagnosis. Diagnosis is still imprecise, expensive and there is not a standard model. We investigated the publications on ZIKV diagnostics and analyzed varieties of diagnostic methods, sensibility, specificity, and the evolution of new methodologies. Conducted in accordance with the PRISMA-P statement, three blocks of MeSH terms were assembled group I virus infection; group II diagnostic methodologies; group III characteristics and varieties on diagnostic methods. Passive smoking at a workplace (or public facilities) was assessed as a secondary measure. MATERIALS AND METHODS In the Japan Public Health Center-based study (baseline 1990 or 1993), we followed 25,391 never-smoking women aged 40 to 69 years old and without diabetes. Passive smoking was defined as having a husband who was a self-reported smoker, and the exposure at a workplace (or public facilities) was self-reported by women. The development of diabetes was identified in questionnaires administered at the 5-year and 10-year surveys. A pooled logistic regression model was used to assess the association between passive smoking and the development of diabetes with adjustment for age and possible confounders. RESULTS Compared to women whose husband had never smoked, women whose husband smoked ? 40 cigarettes/day had a signWe read with interest "The Dallas Consensus Conference on Liver Transplantation for Alcohol Associated Hepatitis"1 , which strove to examine various aspects of early Liver Transplant (LT) for alcohol hepatitis (AH) by defining terms, outlining criteria and identifying issues related to early LT for AH, including payer coverage. Our patients are experiencing real life issues with payer coverage. In our program, only one out of 7 patients evaluated for transplant in our acute AH protocol is determined to be a candidate for transplant listing, which speaks to our strict criteria. Unfortunately, at our institution, patients who met criteria to list were subsequently denied transplant by their insurance. We would like to highlight three recent examples. Therefore, the aim of this retrospective study was to compare perioperative outcomes of emergency surgery to those of elective surgery after reduction of IH. METHODS A total of 112 patients were preoperatively diagnosed with IH between January 2010 and April 2019. Patients were divided into an emergency group (76 patients underwent emergency surgery 21 patients received intestinal resection and 55 patients did not) and a reduction group (36 patients underwent elective surgery after reduction and none required intestinal resection). The outcomes between the groups were compared. A subgroup analysis was also performed on the patients who did not require intestinal resection. RESULTS In patients who did not undergo intestinal resection, the post-operative length of stay was significantly shorter in the reduction group than in the emergency group (8.0 verBACKGROUND Sleep problems are common in bipolar disorders (BDs). To objectively characterize these problems in BDs, further methodological development is needed to capture subjective insomnia. AIM To test psychometric properties of the Athens Insomnia Scale (AIS), and associations with actigraphy-derived measures, applying modifications in actigraphy data processing to capture features of perturbed sleep in patients with a BD. METHODS Seventy-four patients completed the AIS and the Quick Inventory of Depressive Symptomatology, self-report (QIDS-SR-16). Locomotor activity was continuously recorded by wrist actigraphy for ?10 consecutive days. We computed the sleep onset/offset, the center of daily inactivity (CenDI), as a proxy for chronotype, and the degree of consolidation of daily inactivity (ConDI), as a proxy for sleep-wake rhythm strength. RESULTS AIS showed good psychometric properties (Cronbach's alpha = 0.84; test-retest correlation = 0.84, P less then .001). Subjective sleep problems correlated moderately with a later sleep phase (CenDI with AIS rho = 0.
